Position Summary: Under general supervision of the Site Leader, a Direct Care Worker performs duties related to the overall care of consumers in assigned residential facilities. Must be able to successfully complete all of the functions and competencies associated with this position as well as other duties as they may be assigned.

 Cleveland Heights

  • 35.50 hours every 2 weeks = 1st shift (6a-8a) every Friday; (2p-10:30p) every other Monday, Tuesday, Saturday and Sunday (shift length varies between 6 and 6.5 hours)
  • 62 hours every 2 weeks = Mixture of 1st shift (6a-3p) and 2nd shift (3p-11p) throughout the weekdays and every other weekend (shift length 8 hours)

West 153rd

  • 17 hours every 2 weeks = 2nd shift (12p-7p) every other Saturday; (1p-7p) every other Sunday; and (5p-9p) every other Monday (shift length varies between 4 and 7 hours)
  • 48 hours every 2 weeks = 1st shift (7:30a-2:30p) throughout the weekday; 2nd shift (2p-8p) every other Wednesday; and (2p-9p) every other Saturday and Sunday (shift length varies between 6 and 7 hours)

Euclid

  • 38.75 hours every 2 weeks = 2nd shift (2:15p-10:45p) every other Monday and every other Thursday; (3:30p-11p) every Sunday and every other Saturday (shift length varies between 7 and 8.5 hours)

Seven Hills/Independence

  • 22 hours every 2 weeks = 3rd shift (11:30p-7a) every Sunday; 1st shift (8:30a-3:30p) every other Saturday (shift length varies between 7 and 7.5 hours)
  • 18.50 hours every 2 weeks = 1st shift (7a-4:30p) every other Saturday; 2nd shift (12p-9p) every other Sunday (shift length varies between 9 and 9.5 hours)

Parma

  • 16 hours every 2 weeks = 1st shift (7:30a-3:30p) every other Saturday and Sunday (shift length 8 hours)

Essential Job Functions

  • Come to work on time, in the proper attire prepared to work. 
  • Ensure compliance with applicable licensed facility regulations and all required training and certifications are current.
  • Adjust work hours as required for effective operation of the program.
  • Providing consumers with assistance in everyday life skills.
  • Provide active treatment, emotional and health-related services including administering medications, checking vitals to monitor overall health and assisting with medical equipment.
  • Responsible for administrative task associated with providing support.
  • Accurately administering medication and following and maintaining MARS.
  • Immediately report any incidents or accidents involving consumer to the Site Leader.
  • Accurately, legibly and cohesively document all services provided to consumers.
  • Maintain a clean and safe facility and ensure the safety and well being of the consumers at all times.
  • Involve consumers in community and recreation based activities.
  • Assist with and or complete household chores such as cleaning, laundry, cooking, vacuuming, and others.
  • Make and accompany consumers to appointments and community events.
  • Perform and record physical assessments including taking and recording vital signs and medical histories.
  • Exhibit acceptable work standards and immediately correct actions that do not meet Agency standards. 
  • Assist with providing basic personal care and grooming and hygiene, including bathing, basic hair care and getting dressed.
  • Follow Homemaker/Personal Care (HPC) forms and follow goal data sheets. Make sure that ISP’s and Behavior Support Plans are followed.
  • Report UIR and MUI situations to Site Leader according to provided guidelines.
  • Keeping and documenting records of body and general living conditions.
  • Transport consumers in Agency and/or personal vehicles.
  • All other related tasks and projects as assigned by management.

Requirements:

  • Minimum of a High School Diploma, GED or willingness to complete a GED program within 2 years of hire
  • 1 year of work experience preferably in a care providing role, or exposure to individuals with developmental or intellectual disabilities or a combination of experience and education 
  • Generally a minimum age of 18 years of age; limited Direct Support Assistant opportunities available for 16-17 year olds.
  • Basic level of computer proficiency as well as good basic reading and writing skills
  • Excellent communication, interpersonal charting and record keeping skills
  • Ability to accept and provide direction and instruction in a professional manner
  • Successfully pass a background check that includes a BCI and FBI criminal history check
  • Must maintain current Med Pass and CPR/First Aid Certification; training available for CPR and Med Pass onsite; DSA only requires CPR
  • Must have a valid Ohio Driver’s License with less than 6 points
