Intake Process – Licensed Beds

NOTE: Individual must be eligible for services through the County Board of Mental Retardation and Developmental Disabilities or have alternative source of funding.

1. HELP Foundation notifies the County Board of Mental Retardation and Developmental Disabilities (CBMR/DD) of pending vacancies for licensed beds.
2. CBMR/DD staff review caseloads to determine if they have an appropriate referral for anticipated vacancy.
3. Referral packets are sent to Attn: Tamara Honkala, HELP Foundation, 3622 Prospect Ave., Cleveland, Ohio 44115. Packet should include the following: Social History, Physical and Dental Exams, Psychological Report, Psychiatric Records, Financial Information, Current Medications, Current ISP/IEP/Vocational Plan as well as other relevant data.
4. Contact Tami Honkala at 216-432-4810, ext. 339, to arrange for a tour.
5. Situational Assessments are required for HELP Six Chimneys and HELP Housing programs. These are scheduled with the Program Director.
6. Intake Meeting is scheduled.
7. Decision is made regarding our ability to meet needs and/or to require additional information.
